TERMS & CONDITIONS

The following terms and conditions outline the services that Clarke Clinical Psychology Services will provide, the cost of the services provided, when and how you should pay, and what happens if the work is cancelled by either party.

INITIAL DISCUSSION: To arrange an initial free-of-charge discussion lasting up to 45 minutes, you can contact Clarke Clinical Psychology Services via email at [email protected] or by telephone on 07394 685157. During the initial discussion, you will be asked for an outline of the reasons you may need the support of a clinical psychologist. If the Psychologist and you both decide to proceed with a first appointment, the Psychologist may send you (by post/email) a questionnaire to complete, as this will save your time in the first appointment.

REGISTRATION/REFERRAL: If, following initial discussion, you wish to proceed to arranging an appointment with Clarke Clinical Psychology Services, you must complete a referral/registration form with the following details: your name, date of birth, address, insurance details (if appropriate), your NHS general practitioner’s name and practice address, as well as a brief outline of the issues you would like support with. The referral/registration form can be accessed via Clarke Clinical Psychology Services website (here) or it can be emailed/posted to you. You must have the authority to enter into any agreement(s) with Clarke Clinical Psychology Services on behalf of yourself, your child, or your organisation. By completing the referral/registration and by attending appointments, you are providing consent to engage with Clarke Clinical Psychology Services.

CONFIDENTIALITY: Clarke Clinical Psychology Services is registered with the Information Commissioner (Tel: 0303 123 1113) and adheres to the General Data Protection Regulations (GDPR), adhering to the requirement for safe and effective use of personal data, including health and social care information. The Psychologist will respect your confidentiality within certain limitations. It is not possible to cover every possible situation in which challenges about confidentiality might emerge, but they are likely to involve situations where a) there is a significant identifiable risk to you or others, b) there are child protection issues, c) there is a legal requirement to share information with the police and/or court. The Psychologist will take all reasonable steps to keep personal data safe and, before sharing any personal information, the Psychologist will seek your consent. In certain circumstances (e.g., safeguarding concerns or for legal reasons), the Psychologist may have to share your information even if you do not provide consent; if such circumstances were to arise, you would be informed about which information is being shared and the reasons for sharing without your consent.

In all other situations, the Psychologist will only pass on personal information to a third party (usually in a report) when asked to do so by you. If a third party requests a report, the Psychologist will ask for your consent before forwarding the report. The Psychologist normally writes to your GP to inform them about your treatment plan and involve them in your ongoing care. Reports are normally written in collaboration with you.  If you want specific information withheld in a report to your GP (or other third party), you must make this clear before the report is written and forwarded.

Clinical psychologists are required to undergo clinical supervision, during which the Psychologist may discuss your case with another professional (normally another HCPC registered clinical psychologist) who is bound by the same codes of conduct as your Psychologist.

The Psychologist also requires you to abide by confidentiality. You can make an audio and/or visual recording of sessions for your personal use (i.e. for the purposes of 'note taking'. You should discuss recordings with the Psychologist before making any recording. Your sessions are unique to you and, therefore, any material therein must not be used out of context, including being shared with others. The Psychologist cannot accept any responsibility if you share session material with others for whom the material was not intended by the Psychologist, and sharing recordings without consent may be a breach of GDPR legislation.

PAYMENT (SELF-FUNDING): Payment is made at the time of booking assessment, training, consultation or request for report(s). Payment is made via BACS transfer. Bank details will be provided directly to you. In the payment reference section, you must include the name of the person attending the appointments and appointment date (e.g., L McLaughlin 01/05/21). Failure to ensure payment in advance will result in you losing the appointment. At the discretion of the Psychologist, fees for ongoing appointments may be invoiced monthly and this can be discussed at your appointment. For example, some services that take multiple sessions, such as clinical interventions, can be charged monthly via invoice, which will be issued via email, and payment is strictly within seven days of date of invoice. If a payment is not made on time, the Psychologist will send a letter/email requesting payment; each letter/email requesting payment will incur an addition charge of £10. Reminders will be sent weekly for up to one month, after which time, the Psychologist may charge a £50 administration fee if an application to the small claims court is deemed necessary to recover outstanding fees. If it is necessary to instruct a third party to recover the debt, further charges may apply.

PAYMENT (THIRD PARTY): Fees being paid by healthcare insurance providers, lawyers or other third party are negotiated with Clarke Clinical Psychology Services and the relevant third party. Clarke Clinical Psychology Services are registered with a number of healthcare insurance providers, e.g., AXA, BUPA, with each having different conditions about payment, with some having pre-determined fees. As the policy-holder, it is your responsibility to check how much cover your insurance policy provides. The Psychologist will ask you to provide an authorisation code and policy details to proceed with appointment(s). If funding sessions through a healthcare insurance provider, it is your responsibility to ensure that your policy covers payment for services from a clinical psychologist (rather than “counselling” or “CBT therapist” only, as insurers pay different rates to counsellors and CBT therapists). Clarke Clinical Psychology Services charges the “clinical psychology” rate. You must check if there are any amounts not covered by your policy, as you will be responsible for paying any shortfall in fees not covered by your insurance provider.

CANCELLATION OF A SESSION: If you cancel a session less than 48 hours before the appointment, then 100% of the total cost will be payable, unless the session is taken by another patient. Cancellations under 48 hours, or non-attendance at appointment, will likely be counted as one of your appointments by some health insurance company if they are paying. If you reschedule before the 48-hour deadline, any fees already paid can be carried forward to cover the new appointment, which attracts the same terms and conditions for cancellation. Cancellation/rescheduling can be done by email: [email protected] or by telephone: 07394 685157. Time of cancellation will be recorded as the time/date stamp provided by email or telephone call. For the avoidance of doubt, 48 hours is exactly 48 hours, e.g., a session booked for Friday at 3.00pm would need to be cancelled before 3.00pm on the preceding Wednesday to avoid having to pay the late cancellation fee. If more than 48 hours’ notice is given to cancel an appointment, then there will be no cost for that appointment. If the Psychologist cancels the appointment at any time before the start of the appointment, then there will be no charge.

If you have any foreseeable breaks in treatment, e.g., booked holiday, you must advise the Psychologist of the dates in advance. The Psychologist will also advise you of any foreseeable breaks in treatment in advance.

TERMINATION OF A SESSION AND/OR TREATMENT: The psychologist is not required to continue the session beyond the booked finishing time, but may do so, if deemed appropriate. The Psychologist reserves the right to terminate a session, without refund, if the Psychologist believes that you pose a personal risk to the Psychologist or anyone in, or near, the clinic setting. In such circumstances, the appropriate authorities will be notified. If, during treatment, the Psychologist determines that further treatment is not appropriate, then the Psychologist reserves the right to terminate treatment and any remaining sessions that have been prepaid will be refunded.

LATE ARRIVAL: You will be charged for the whole timeslot provided, regardless of late arrival or early departure. For example, if you have booked an appointment from 10.00am to 11.00am and arrive at 10.10am, you will only have 50 minutes remaining, but will be charged for the full hour. If you have not arrived within 20 minutes of the scheduled start time, the Psychologist reserves the right to cancel the session and you will still be charged for the full timeslot.

REPORTS: The Psychologist charges for all written reports, with the price depending on the length of time that it takes to write the report. A quotation for writing a report(s) will be provided via email. Payment for reports are made in advance of the report being written. The Psychologist will agree with you the likely timescale for completing the report (i.e. the date you will receive the report(s)). All reports will be based on the Psychologist’s professional opinion.

FEES: Clinical assessment and interventions are charged at £120 per one hour session, which is payable at each session or invoiced monthly by arrangement.

A full cognitive assessment (and report) starts from £1,020.

Psychological reports are charged at £120 per hour.

One page letter (not full report) requested for employer or university/school is £35

Supervision of clinical psychologists is £85 per hour.

Training is £795 per day (plus travel and expenses, e.g. training materials).

School observation is £275 per session (morning or afternoon session).

Travel time is £40 per hour, plus £0.45 per mile.

EMAIL/TELEPHONE POLICY: The Psychologist may receive email communication, including letters, assessments and reports from third parties authorised by you. Reviewing these documents will attract a fee based on the agreed hourly fee. Telephone calls will also attract a fee based on the agreed hourly fee. As the Psychologist can experience a high volume of emails/telephone calls, a rapid response cannot be guaranteed, but the Psychologist will aim to respond within a reasonable timeframe.

DESIRED OUTCOMES/GOALS: Whilst the Psychologist will make all reasonable efforts, paying attention to the evidence base to help you reach the desired outcome, it is not the Psychologist’s responsibility to ensure that the desired outcome(s) is achieved. You take full personal responsibility for this and the Psychologist cannot provide a guarantee of a successful outcome and refunds are not provided where treatment has not been successful in reaching your desired outcome. If a diagnostic assessment is completed and a diagnosis that you expected is not clinically indicated, refunds will not be given, but you will receive a full report explaining the assessment process and the reasons for clinical opinion.

HCPC REGISTRATION: The clinical psychologist you work with will be registered with the Health and Care Professions Council, which is the regulator for practitioner psychologists and whose primary role is to protect the public. Information about the HCPC can be found at: https://www.hcpc-uk.org/, where you can also check to ensure that your psychologist is on the register.

COMPLAINTS: The Psychologist will be happy to handle complaints in person or via phone but, if you prefer, complaints can be sent via email. The Psychologist will aim to respond to your complaint within fourteen days.

INSURANCE: The Psychologist has insurance that provides professional indemnity of up to £1,500,000 and public liability indemnity of up to £10,000,000.
